How to Request Use of UASD Schools Facilities

Uniontown Area School District Building Permit Procedures

  • All requests are to be submitted at least two (2) weeks prior to the event for processing.
  • Every date that is being requested must be written out on the permit or on a separate document that can be attached with the permit.
  • Each activity must end by 8:00 p.m.
  • The Building Request Form must first be submitted to the building principal of the facility being requested.


  1. Click on the link for the UASD Request for Use of School Facilities Form.
  2. Complete the form.
  3. Provide an email address on the request form for the processed permit copy to be returned.
  4. Save it to your files.
  5. Please attach any additional documents if needed in the email.
  6. Email it to the appropriate building-level principal (listed below) for approval.
